Based on the book The Bell Witch: An American Haunting, by Brent Monahan, An American Haunting tells the story of a noisy, cruel spirit known both as "Old Kate". In 1818, she decided to "move in" with the John Bell family, where she spent her time making knocking noises and gnawing sounds. Soon, though, she found a voice. With her newfound powers, she tormented the Bells and eventually became the only documented spirit in U.S. history to cause a man's death.

A local schoolteacher, Richard Powell, witnessed these strange events and eventually recorded them on paper for his daughter and posterity. In the film, Powell is portrayed by James D'Arcy, while the Bells are headed up by Donald Sutherland as the patriarch and Sissy Spacek as his wife, and Rachel Hurd-Wood as the daughter, a particular target of the spirit.
